Properties:

A broad-spectrum antibiotic, a derivative of phosphonic acid. The mechanism of action is associated with the suppression of the first stage of bacterial cell wall synthesis. Being a structural analogue of phosphoenolpyruvate, it competitively and irreversibly inhibits the enzyme UDP-N-acetylglucosamine enolpyruvyl transferase, which catalyzes the reaction of forming UDP-N-acetyl-3-0-(1-carboxyvinyl)-0-glucosamine from phosphoenolpyruvate and UDP-N-acetyl-D-glucosamine. Fosfomycin is also able to reduce bacterial adhesion to the mucous membranes of the bladder, which can play the role of a predisposing factor for recurrent infections.

Fosfomycin is active against gram-positive aerobes. Secondary resistance of microorganisms to fosfomycin develops rarely. There is no cross-resistance with other antibacterial agents. When combined with beta-lactams, fluoroquinolones, glycopeptides or aminoglycosides, there is a pronounced synergism of antimicrobial action against a wide range of gram-positive and gram-negative aerobic bacteria.

Indications:

Bacterial urinary tract infections of various localizations caused by microorganisms sensitive to fosfomycin:

– acute uncomplicated urinary tract infections; asymptomatic bacteriuria;

– prevention of urinary tract infections after surgery and transurethral diagnostic studies.

Method of administration and dosage:

Inside.

Monuril is used once, on an empty stomach, 2-3 hours before or after meals, preferably before bedtime, after emptying the bladder: adults and children over 12 years old in a daily dose of 3 g. The contents of one packet are dissolved in approximately 150-200 ml of water and taken immediately after preparation.

The course of treatment is 1 day.

For the prevention of urinary tract infections during surgical interventions and diagnostic procedures, the specified dose is taken 2 times – 3 hours before the intervention and 24 hours after it.

Contraindications:

Do not take for people weighing less than 50 kg.

Hypersensitivity to fosfomycin or other components of the drug; severe renal failure.

Precautions:

The occurrence of acute and persistent diarrhea during or after antibiotic treatment may be a symptom of antibiotic-induced colitis. The most serious form is pseudomembranous colitis.

The product contains sucrose. Patients with rare hereditary diseases, fructose intolerance, or sucrase-isomaltase deficiency should not take this product.

Simultaneous food intake slows down the absorption of granules, therefore it is recommended to take the drug on an empty stomach, 2-3 hours before or after meals.

Side effects:

From the nervous system: Frequent: headache, dizziness, asthenia.

From the gastrointestinal tract: Uncommon: diarrhea, nausea, dyspepsia.

From the liver and biliary tract: Uncommon: increased activity of “liver” transaminases.

From the genitals and mammary gland: Uncommon: vulvovaginitis.

From the blood and lymphatic system: Uncommon: thrombocytosis, leukopenia.
