Properties:

Stimulator of gonadotropin production. A product with estrogenic and antiestrogenic activity (dose-dependent effect). It has a non-steroidal structure, which is close to chlorotrianisene. It is believed that the action of clomiphene is due to specific binding to estrogen receptors of the hypothalamus and the hypothesis that it increases the secretion of gonadotropic hormones of the pituitary gland. This in turn stimulates the maturation and endocrine activity of the follicle in the ovary, as well as the subsequent formation and stimulation of the function of the corpus luteum. In high doses, it inhibits the secretion of gonadotropins. It does not have gestagenic and androgenic activity.

Indications:

Anovulatory infertility (ovulation induction), amenorrhea (dysgonadotropic form), secondary amenorrhea, post-contraceptive amenorrhea, Stein-Leventhal syndrome (polycystic ovary syndrome), oligomenorrhea, galactorrhea (against the background of a pituitary tumor), Chiari-Frommel syndrome (prolonged postpartum amenorrhea-galactorrhea syndrome), androgen deficiency, in men – oligospermia.

Method of administration and dosage:

The optimal dosage regimen is prescribed by a doctor. It is necessary to strictly observe the compliance of the used dosage form of a specific drug with the indications for use and dosage regimen.

Taken orally.

To stimulate ovulation, prescribe 50 mg once daily before bedtime, starting from the 5th day of the menstrual cycle, for 5 days (at any time if there is no cycle). If there is no effect (ovulation does not develop within 30 days), increase the dose to 150 mg/day or extend the course to 10 days. The course dose should not exceed 1 g. The development of ovulation is determined by the presence of a biphasic basal temperature, an average cyclic increase in LH production, an increase in serum progesterone during the probable mid-luteinization phase or during menstrual bleeding in women with amenorrhea. If ovulation occurs and there is no pregnancy, then the same dose should be repeated during the next course of treatment. If menstrual bleeding does not follow probable ovulation, the possibility of pregnancy should be taken into account and this possibility should be excluded before a new course of treatment.

Men are prescribed 50 mg 1-2 times a day for 3-4 months (systematic monitoring of spermogram is required).

Contraindications:

Hypersensitivity to clomiphene and other components of the drug.

Renal failure.

Liver failure.

Pregnancy and lactation period.

Ovarian cyst (except PCOS).

Tumor, pituitary hypofunction.

Thyroid dysfunction.

Disorders of adrenal function.

Metrorrhagia of unknown etiology.

Visual impairment.

Precautions:

Clomiphene increases the risk of multiple pregnancy.

Before using clomiphene, a thorough gynecological examination should be performed.

The use of clomiphene is indicated in cases where the total level of gonadotropin in the urine is below the lower limit of normal or at the normal level, palpation of the ovaries does not reveal any deviations from the norm, and the functions of the thyroid gland and adrenal glands are also normal.

During the treatment period, caution must be exercised when driving vehicles and engaging in other potentially dangerous activities that require increased concentration and quick psychomotor reactions.

Side effects:

From the nervous system: dizziness, headache, drowsiness, slowing of mental and motor reactions, depression, increased excitability, insomnia.

From the digestive system: nausea, vomiting, gastralgia, flatulence, diarrhea, acute abdomen syndrome.

Allergic reactions: rarely – rash, allergic dermatitis, vasomotor disorders.

From the genitourinary system: rarely – polyuria, increased urination, pain in the lower abdomen, dysmenorrhea, menorrhagia, enlarged ovary (including cystic), rarely – vaginal dryness.

Others: weight gain, flushing of the face, rarely – decreased visual acuity, photophobia, alopecia, pain in the mammary glands.

The appearance of signs of ovarian hyperstimulation and general side effects requires a reduction in dosage or discontinuation of the drug.
