Properties:
Domperidone is a dopamine antagonist with antiemetic properties. Domperidone poorly penetrates the GERD. Extrapyramidal side effects are rare with domperidone, especially in adults, but domperidone stimulates the release of prolactin from the pituitary gland. Its antiemetic action may be due to a combination of peripheral (gastrokinetic) action and antagonism of dopamine receptors in the chemoreceptor trigger zone, which is located outside the BBB.
When administered orally, domperidone increases the duration of antral and duodenal contractions, accelerates gastric emptying, and increases lower esophageal sphincter pressure. Domperidone has no effect on gastric secretion.
Indications:
To relieve symptoms of nausea and vomiting.
Method of administration and dosage:
The drug is taken orally. It is recommended to take Motil Fast sachets 15-30 minutes before meals; if the drug is taken after meals, the absorption of domperidone may slow down.
Adults and adolescents over 12 years of age and children weighing ≥35 kg – 1 sachet (10 mg) 3 times a day, the maximum daily dose is 3 sachets (30 mg).
Children under 12 years of age and weighing 35 kg or more – 1 sachet (10 mg) 3 times a day, the maximum daily dose is 3 sachets (30 mg).
Contraindications:
– hypersensitivity to domperidone or any other component of the drug;
-prolactinoma;
– concomitant use of oral forms of ketoconazole, erythromycin or other potent inhibitors of the CYP3A4 isoenzyme that cause prolongation of the QT interval, with the exception of apomorphine;
– severe electrolyte disturbances or heart disease such as chronic heart failure;
– gastrointestinal bleeding, mechanical intestinal obstruction, perforation of the stomach or intestines;
– moderate to severe liver failure;
– body weight less than 35 kg;
– children under 12 years of age with a body weight of less than 35 kg;
-pregnancy;
– the period of breastfeeding.
With caution: childhood, impaired renal function.
Precautions:
Domperidone is not recommended for the prevention of nausea and vomiting after anesthesia.
During long-term therapy with the drug, patients should be under regular medical supervision.
Domperidone may cause prolongation of the QT interval on the electrocardiogram. In postmarketing studies, prolongation of the QT interval and occurrence of torsades de pointes have been reported in rare cases in patients taking domperidone. These adverse reactions were noted mainly in patients with risk factors, with significant electrolyte disturbances or concomitantly taking drugs that prolong the QT interval.
Side effects:
Mental disorders: uncommon – increased excitability, nervousness.
From the nervous system: often – dizziness; rarely – convulsions; frequency unknown – extrapyramidal disorders.
From the cardiovascular system: frequency unknown – ventricular arrhythmia*, ventricular tachycardia of the “pirouette” type, sudden coronary death*.
From the gastrointestinal tract: frequency unknown – dry mouth.
From the skin and subcutaneous tissues: frequency unknown – angioedema.
From the kidneys and urinary tract: uncommon – urinary retention.
Laboratory and instrumental data: uncommon – abnormalities in laboratory parameters of liver function; rare – hyperprolactinemia.
Storage method:
Store at a temperature not exceeding 25 degrees in a dry place.
