Properties:

A drug for the treatment of erectile dysfunction, a PDE5 inhibitor.

Penile erection is a hemodynamic process based on the relaxation of the smooth muscles of the cavernous bodies and the arterioles located in it. During sexual stimulation, nitric oxide (NO) is released from the nerve endings of the cavernous bodies, activating the enzyme guanylate cyclase, which leads to an increase in the content of cyclic guanosine monophosphate (cGMP) in the cavernous bodies. As a result, the smooth muscles of the cavernous bodies relax, which helps increase blood flow to the penis.

Vardenafil blocks PDE5, which is responsible for the breakdown of cGMP, as a result of which the action of endogenous NO in the cavernous bodies during sexual stimulation is enhanced, which determines the ability of vardenafil to enhance the natural response to sexual stimulation.

The clinical effect is realized even before reaching Cmax. After oral administration in doses of 20 mg and 10 mg, the onset of action is after 10 minutes, which provides an erection sufficient for penetration and successful completion of sexual intercourse in 34% and 40% of patients with mild and moderate erectile dysfunction, respectively. After 25 minutes, the effect occurs in 53% and 50% of patients, respectively, which coincides in time with the onset of the active substance in the blood and the rapid increase in its concentration. Duration of action is 8-12 hours.

Indications:

Erectile dysfunction (inability to achieve and maintain an erection sufficient for sexual intercourse).

Method of administration and dosage:

The maximum daily dose of Powerect tablets is 20 mg (1 tablet).

25-60 minutes before expected sexual activity, take 1 tablet with a sufficient amount of water.

Vardenafil can be taken with or without food. Sufficient sexual stimulation is required for treatment to be effective.

Contraindications:

Concomitant therapy with nitrates or drugs that are nitric oxide donors; childhood and adolescence under 18 years of age; hypersensitivity to vardenafil; loss of vision in one eye due to anterior ischemic optic neuropathy of non-arteritic origin, regardless of whether it is associated with the use of PDE5 inhibitors; Medicines for the treatment of erectile dysfunction, including sildenafil, should not be used in men for whom sexual activity is not recommended (for example, patients with severe cardiovascular diseases, such as unstable angina or severe heart failure).

Safety in the following groups of patients has not been studied, therefore use is contraindicated in: severe liver and kidney dysfunction, arterial hypotension (BP less than 90/50 mm Hg), history of myocardial infarction (in the last 6 months), unstable angina; in hereditary degenerative diseases of the retina (for example, retinitis pigmentosa), co-administration with ritonavir, indinavir and some other drugs.

Precautions:

Because sexual activity is associated with a certain degree of cardiac risk, the patient’s cardiovascular status should be assessed before prescribing vardenafil for erectile dysfunction.

Vardenafil is not intended for use in children.

Impact on the ability to drive vehicles and operate machinery:

Before use in patients whose activities involve driving vehicles and operating machinery, it is necessary to determine their individual reaction to vardenafil.

Since vardenafil has not been used in patients with a tendency to bleeding and in patients with exacerbation of peptic ulcer disease, the drug can be prescribed in these cases only after a careful assessment of the risk-benefit ratio of therapy.

Use during pregnancy and breastfeeding:

The drug is not used in women.

Side effects:

From the nervous system: headache, dizziness are possible; rarely – increased muscle tone.

From the cardiovascular system: possible facial flushing; rarely – increased blood pressure, decreased blood pressure, tachycardia.

From the digestive system: possible – dyspepsia, nausea.

From the respiratory system: nasal congestion is possible.

Dermatological reactions: rarely – photosensitivity, allergic reactions.

From the reproductive system: rarely – priapism, hematuria.

Infectious diseases: rarely conjunctivitis.

From the organ of vision: uncommon – impaired color vision, hyperemia, pain and discomfort in the eyes, photophobia.
